Role overview Serve as the frontline voice for patients enrolling in a technology-enabled chronic care programme, conducting high-volume inbound and outbound phone outreach to educate, engage, and support eligible participants. The role blends patient education, scheduling coordination, and meticulous documentation, acting as the bridge between patients, providers, and care teams. It begins fully remote and will eventually move onsite to a new Indianapolis office once construction is complete.
Responsibilities - Make high-volume inbound and outbound calls to educate patients on the benefits of proactive chronic care support and guide them through enrolment - Explain programme purpose and value clearly, build rapport, and professionally address questions, concerns, and objections - Manage the Scheduling inbox by responding to patient inquiries, triaging requests, routing clinical concerns, and ensuring timely follow-up - Document every interaction, outreach attempt, and enrolment outcome accurately and promptly in the system of record - Collaborate with client care teams, providers, and internal partners to maintain continuity of care and a seamless patient experience - Follow established outreach workflows, scripts, quality standards, and compliance requirements while adapting conversations to each patient - Escalate patient concerns, engagement barriers, and operational issues, contributing to process improvements that lift enrolment success
Requirements - Bilingual in English and Spanish - Prior care management experience - Excellent written and verbal communication skills, especially over the phone, with strong telephone etiquette and call centre experience - Ability to sustain high-volume, high-quality patient outreach with professionalism and consistency - Friendly, personable, and self-motivated approach with a passion for helping patients with chronic conditions - Strong organisational skills, attention to detail, and commitment to accurate record-keeping - Comfort with web-based tools, reliable internet access, plus a personal laptop and smartphone
Nice to have - Experience in sales, outreach, or upselling products and services - Background in healthcare, care coordination, patient engagement, or a startup environment
Benefits and work setup - Part-time, permanent role at 30 hours per week (6 hours per day, Monday to Friday, 9:00am to 3:00pm PST) - Pay rate of $21.00 per hour on a W-2 basis, plus shift differential - Initially fully remote, transitioning to onsite at a new Indianapolis office once it opens; not eligible for visa sponsorship
