Clinical Imaging Platform Engineer

Role overview This role sits at the intersection of product engineering and infrastructure for a clinical AI radiology platform. The mission is to maximize label quality and reader trust by owning the imaging viewer and annotation system that radiologists use every day. It blends medical-imaging UX with a self-instrumented annotation pipeline whose throughput drives downstream model performance.

Responsibilities - Build and maintain a DICOM viewer with volume rendering, stack-rendering fallback, on-demand multiplanar reformats, and progressive loading from the first portion of each HTJ2K codestream - Handle radiological correctness that only surfaces in production: MONOCHROME1 inversion, left/right orientation, rulers under gantry tilt, and signed-pixel codec mismatches - Develop a 3D mask annotation system with AI-assisted click-to-segment across axial, coronal, and sagittal planes, plus classical tools such as region growing, FWHM thresholding, and multi-seed refinement - Operate the annotation evaluation pipeline: case-pool ledgers, per-annotator phase state machines, ground-truth and peer-overlap batches, and per-lesion Dice agreement scorecards across readers working from non-matching geometries - Resample across series via NIfTI affines and store masks with RLE so the workflow stays reproducible - Reduce reader friction so annotation throughput translates directly into stronger model performance

Requirements - Hands-on experience with medical-imaging libraries such as Cornerstone3D and VTK.js - Working knowledge of DICOMweb, HTJ2K, NIfTI, and RLE formats - Track record of building interfaces used for hours each day, watching real users, and iterating from those observations - Familiarity with segmentation algorithms including region growing, thresholding, and multi-seed refinement - Care for edge-case correctness in clinical reading workflows - Comfort with written, asynchronous code review in a distributed team

Nice to have - Background in regulated, safety-critical, or clinical software

Benefits and work setup - US, Boston hybrid: $140,000–$180,000 base plus approximately 10% discretionary bonus; equity may be offered to top candidates - International, remote: cash-weighted base tied to the local market; contractor or EOR employment depending on country; cash-only with no equity; no visa required - Async written reviews across time zones; international engineers own the same repositories, data, and review authority as the rest of the team
